How is an ankle unlike a consequence?

If a person gains a lot of weight their ankles become less pronounced because the calf starts sagging down into the ankle. It's a cankle. That would seem like a consequence, but it isn't because the cankle still works, which is why an ankle is unlike a consequence...?
