| Introduction | I am simply a daughter who set out to help my dad organize his medical information after he was diagnosed with esophageal cancer in 2008. Because I could not find anything that met our needs on the market, I created a Care Management Journal of our own. My thought was that if this was a helpful tool for our family, then maybe it would be helpful to others who were faced with the same daunting task of keeping up with medical information. My mom came up with the name, Be Patient Ready. I found it a perfect name as it is proactive. As the patient, you are forced to be patient but also to be ready! I pray that this will be a tool which will help you, your loved one or friend to move forward confidently in your care management. |
