This microcode implementation difficulty is indeed head scratching and your investigation skills are just awesome.

I bumped to a very interesting resources on chips details (https://www.oguchi-rd.com) including development papers, analyses and hand made masks design process with detailed notes made available by Mr. Tetsuji Oguchi, apparently the author of many chips himself.

There are detailed die photos of many chips also available on his page. It is interesting to see one of the official(?) 8086 copy by OKI - 80C86A variant from 1985 with various modifications:
