At the very least you should play the Happy Birthday song in your office, very low.
Then when the patients ask if you can hear it, say "No, but it IS my birthday, how odd. Do you always hear occasion-appropriate music in your head?"
Bang! You just got yourself 30-40 more visits from the patient as a present.
